If paronychia shows signs of suppuration and pus swelling is severe, a surgical incision may be made to drain the pus. The incision may be made on both sides of the nail groove, and then a longitudinal incision may be made to drain the pus. If there is subungual empyema, consider removing part or even all of the nail.
