The most effective drugs for preventing cerebral thrombosis are antiplatelet agents. Antiplatelet agents are recommended for most patients with noncardiac ischemic stroke and tia. Antiplatelet agents include aspirin, clopidogrel, aspirin dipyridamole extended-release, and cilostazol. For patients with recent ischemic stroke or tia, dual antiplatelet therapy with aspirin and clopidogrel may be considered if severe intracranial atherosclerotic stenosis (70% to 99%) is present, and the duration of dual antiplatelet therapy should not exceed 3 months. Prolonged dual antiplatelet therapy is not recommended for the general population.
