The anteroposterior position of the uterus is normally not easily changed. The uterus is located in the middle of the female pelvis. It may be anterior, posterior, or horizontal. Either position is normal. The anterior uterus refers to the forward position of the uterus and the posterior position of the cervix points to the posterior fornix of the vagina. The guard uterus refers to the backward position of the uterus and the upturned position of the cervix. The normal position of the uterus depends on the four pairs of ligaments of the uterus, including the round ligament, the broad ligament, the main ligament, and the uterosacral ligament, and on the rest of the pelvic floor muscles and fascia. If there is any cause of pelvic floor tissue destruction or dysfunction, it may change the position of the uterus and cause uterine prolapse.
