1. The

2. Coronary artery spasm

3. Myocardial bridge

If the heart develops abnormally during embryonic period, a myocardial bridge is formed. During cardiac contraction, the myocardial bridge is compressed, resulting in occlusion of a segment of coronary artery in myocardial bridge, thus causing myocardial ischemia.
