High horseshoe fistulas occur at higher locations and are usually caused by local abscess rupture. Patients can generally use potassium permanganate solution for hip bath and apply golden cream externally. Penicillin or cephalosporins may also be used during acute attacks, but not for too long. Surgery is needed to cut open the fistula and remove surrounding scar tissue if necessary.
