Chocolate cysts are cysts associated with endometriosis. They form in the pelvis, most often due to endometrial tissue growing outside the uterus. These ectopic endometrial tissue is also affected by hormones during menstruation, causing bleeding and inflammation, which can cause pelvic pain. The pain of chocolate cysts is usually associated with the menstrual cycle, may worsen before or during menstruation, may be intermittent, intense, and may spread to the lower abdomen, lower back, and pelvic areas.
