A throbbing pain in the right back of the head can be seen in the third occipital neuralgia. The third occipital neuralgia is unilateral occipital and suboccipital pain. It is the main symptom of patients with neck pain. Common causes include upper cervical spondylosis, spinal canal diseases, atlanto-occipital deformities, spinal tuberculosis, spinal cord tumors, osteoarthritis, metastatic tumors, upper respiratory tract infections, tonsillitis, influenza, rheumatism, diabetes, and neck trauma. It is especially seen in whiplash injuries. The incidence rate can reach 27%. Percutaneous block of the third occipital nerve is diagnostic if neck pain and headache disappear. Headache is caused by degenerative disease of the C2-C3 intervertebral joint or compression of the C3 nerve branch by posttraumatic joint disease.
